Dilution is recommended if a patient experiences initial severe side effects (such as headache, depression, or body aches) even from the "ultra low dose" spray. Starting with a Very Low Dose (VLDN) ensures a slow introduction to minimize potential sensitivities or initial reactions.
For certain conditions like Lupus, Doc Frank recommends taking one spray below the tongue in the morning. However, some protocols suggest taking LDN at night to align with the body’s natural endorphin cycle, which may minimize potential sleep disruption. It is generally advised not to take the dose later than 2:00 PM if sleep disturbances occur.
Clinical studies show that LDN can significantly reduce daily pain and improve quality of life in patients with fibromyalgia. Anecdotally, some users report "wonderful results" early on, though careful titration is necessary to manage potential initial side effects.
Naltrexone exists in two forms (isomers). While the L-isomer primarily binds to opioid receptors, the D-isomer is thought to have distinct binding properties on immune cells, contributing to LDN's diverse effects on immune regulation and inflammation, independent of the opioid receptors.
Scientific research suggests that naltrexone at low doses may modulate the activity of glial cells (immune cells in the central nervous system) by antagonizing Toll-like Receptor 4 (TLR4). This mechanism helps reduce neuroinflammation and central sensitization, making it relevant for various chronic pain and neurological conditions.
LDN is known to temporarily inhibit the binding of Opioid Growth Factor (OGF)—a peptide that regulates cell proliferation—to its receptor (OGFr). This modulation is hypothesized to be a factor in LDN's potential anti-inflammatory and anti-proliferative effects, particularly in autoimmune and cancer contexts.
Early clinical and preclinical research suggests that LDN may possess anticancer effects by modulating immune responses and promoting apoptosis (programmed cell death) in cancer cells. It is being explored as an adjunctive therapy alongside conventional treatments.
Naltrexone is an opioid antagonist typically used at standard, higher doses (e.g., 50 mg) to treat opioid and alcohol dependence. Low Dose Naltrexone (LDN) refers to significantly lower daily doses, generally ranging from 0.5 mg to 4.5 mg. These low doses are thought to have immunomodulatory and anti-inflammatory effects rather than full, sustained opioid receptor blockade.
Doc Frank's products are available in sprays:
Available as "ultra low dose naltrexone" (ULDN), "Low Dose Naltrexone" (LDN) sprays and Very Low Dose Naltrexone (VLDN), often with peppermint or plain flavors.
Strengths are approximately 100 mcg (0.1 mg) per spray.
LDN is widely investigated for conditions involving immune system dysregulation and chronic inflammation. This includes Crohn's disease, multiple sclerosis (MS), Hashimoto's thyroiditis, fibromyalgia, and Lupus, often showing benefits in reducing symptom severity and improving quality of life.
LDN, especially compounded or diluted solutions like VLDN, generally requires refrigeration to maintain potency and stability. The instructions for making VLDN specifically advise keeping the syringe containing the concentrated liquid in the fridge for future use.
While generally well-tolerated, initial side effects can include sleep disturbances (such as vivid dreams or insomnia), headaches, nausea, anxiety, or mood changes. These are often transient and can be mitigated by slowing the titration or diluting the dose (VLDN).
Due to the very low dosage and transient receptor blockade, LDN is not associated with the development of physical dependence or significant withdrawal symptoms upon abrupt cessation, unlike standard dose Naltrexone used for addiction or conventional opioids.
The time to efficacy varies greatly depending on the condition. While some may report initial positive responses quickly (e.g., the first day/night for fibromyalgia), significant therapeutic benefits for chronic and autoimmune conditions often take several weeks or months (e.g., 3 to 6 months) of consistent use.
The dilution protocol involves emptying the content of the LDN spray bottle into a sterile syringe, injecting 1 ml of the original liquid back into the spray bottle, and filling the rest with boiled, cooled water. This method yields a VLDN solution of approximately 100 mcg per spray.
Doc Frank's protocols include LDN (typically 1 spray below the tongue in the morning) for both Long COVID and Lupus. For Lupus, it is often paired with supplements like Alpha Lipoic Acid (ALA). For Long COVID, anecdotal reports suggest LDN helps with symptoms like fatigue, brain fog, and poor sleep, leveraging its anti-inflammatory properties.
LDN is believed to function as a short-acting, transient opioid receptor antagonist. This brief blockade signals the body to increase its own production of endogenous opioids (endorphins and enkephalins) and up-regulate opioid receptors. This compensatory increase in natural opioids is hypothesized to regulate immune function, reduce chronic inflammation, and enhance feelings of well-being.
LDN can trigger a feeling of "satisfaction and overall contentment," which helps reduce cravings. While noted as less effective for potent weight loss than modern drugs, LDN avoids the risk of lifelong dependence and potential weight rebound upon cessation associated with those newer medications.
If a patient requires opioid analgesia for surgery, LDN is typically advised to be stopped 72 hours prior to the procedure and restarted 24 hours after the last opioid dose to ensure the pain medication is fully effective.
Doc Frank states that LDN "will not interfere with any of your medications or supplements." However, general medical advice strictly contraindicates taking LDN concurrently with full-agonist opioid pain medications (e.g., codeine, morphine), as LDN will block their effects and may precipitate withdrawal.

I would like to thank you for the incredible service and wonderful product you supply. I take LDN for arthritis and sciatica pain and it works just swell, if not better than the Diclufenac I took for an extended period and which caused irreversible kidney damage. I have already recommended this product to a couple of other people and hope it brings them the same level of relief it brought me. Oh!! And the psychological effect it has had is almost as profound, calmness, more focused and positive. Thanks once again.

Integrows sublingual LDN spray is the most helpful ‘treatment’ for my Fibromyalgia, I wake up feeling positive about the day ahead – it gives me a boost and changes how my day plays out. I recommend LDN to anyone suffering from any chronic ailment, whether cancer, diabetes, crohns, IBS, and the list goes on – it works by bringing the body into alignment through positive signaling via enhancing neurotransmitters or neurochemicals – dopamine and serotonin. Due to its mode of activity – it helps deal with a multitude of ailments through positive signaling mechanisms. Highly recommended.
